Transitions Hub

The Transitions Hub supports young people across the Liverpool City Region who have an Education, Health and Care Plan (EHCP) and are working at Entry Level 2 and above, helping them prepare for adulthood, further education, or employment.

Whether our learners are moving on from a specialist school or seeking a stepping stone to general further education, the Transitions Hub is designed to bridge the gap.

Who is it for?

We welcome learners aged 16–25 from Sefton, Liverpool, Wirral, Halton, St Helens, and Knowsley who:

Are working at Entry Level 2 and above
Have an EHCP (Education, Health and Care Plan)
Would benefit from a smaller, more supported environment
Have social, emotional and mental health needs that are currently a barrier to them accessing general further education
Are aiming for future progression into college, employment, or supported internships
Need to build confidence, soft skills, and independence

Five-day provision

Designed around you

Every element of the Transitions Hub has been thoughtfully crafted to support our learner's education, wellbeing, and personal growth.

Learners will study Maths and English qualifications appropriate to their assessed level
Learners will apply their skills such as Maths and English, daily living skills and food hygiene into entrepreneurial activities such as community café events
Modern classrooms
A kitchen/home area to develop life skills such as cooking, cleaning and washing
Wellbeing and de-escalation garden space
Accessible facilities
